Noticeboard

The CCG have now recommenced the Protected Time for Learning sessions, therefore all practices will be closed from 12 noon on Thursday 17th June. We will reopen on Friday 18th June at 8am. If you require urgent assistance during our closure then please contact NHS 111. Many thanks

From 17 May 2021, people in England who have been fully vaccinated against COVID-19 can demonstrate their vaccination status for international travel. A full course is currently two doses of any approved vaccine. Vaccine status will be available from:

  • the NHS App which you can download from app stores

It may take more than a week for your identity to be checked and verified so you can use this service.

If you cannot access this online service, and you have had 2 vaccines, you can request a paper letter from the NHS by calling 119. Only call 119 if you are due to travel abroad in the near future and have had your second dose more than 5 working days ago. It may take 7 working days for the letter to arrive.

This practice is not able to provide you with a letter that shows your COVID-19 vaccination status.

When you're planning your travel, check the latest information on demonstrating your COVID-19 status when travelling abroad on the gov.uk website. Make sure there is time to get  proof of your vaccination status before you leave.

New Patient Registration

If you wish to join the Practice, please bring your medical card (if available) with you to the Reception desk. You will be asked to complete the necessary registration form, which will enable us to obtain your medical records.

If you wish to complete a registration form prior to attending the surgery please fill in the pre-registration form found here and the Holderness Health registration form here Please print both forms and post it to the Practice.

You have a right to express a preference to receive services from a particular person, or type of clinician (either generally or in relation to a particular condition). Your request can be given either verbally or in writing to the Receptionist. We will then endeavour to comply with any reasonable request.

All newly registered patients are offered a health check appointment, within six months of joining the Practice. This gives the Nurse/Doctor a chance to get a brief medical history prior to receiving the patient’s records. Medical treatment is available from the date of registration.

Temporary Residents

If a visitor to your home needs to see a Doctor, you may make arrangements for them to be seen at the Practice on a temporary basis.
